I'm bored in my relationship?

We've been together for 2 years and i'm always the one who has to come up with activities. If it's left to him all we do is just stay at each others houses. He seems to be incapable of coming up with anything unless i ask him to and i'm really fed up with having to act like a mother to him, constantly reminding him of everything. a good example of this is that he completely forgets about all occasions like my birthday, valentines etc. and doesn't make up for it. sure, guys will insist that they forget stuff like that but if a girl can organise herself enough to remember then so can a guy. i told him about this and he tried to make changes but he's still as unreliable as ever. he tried to change this but i feel as if it's too late for him to change my opinion about him, it just took too long for him to react. i also know that he would be absolutely devastated if i left him cuz he has nobody else but me. what should i do?
